The Current Event Summary Form opens by asking for the title and source of a news article or event, then its publication or occurrence date — establishing the core facts before anything else. A required open-ended summary question then captures the key points and significance, followed by a rating question on how relevant the event is to the respondent's field, giving the form owner a quick sense of priority at a glance. Two optional questions — one asking for a perceived impact level (from 'Low Impact' to 'Uncertain') and one for keywords or tags — help with filtering and organizing submissions later without blocking anyone who's short on time. A final optional name field lets submitters identify themselves if they choose. After submitting, respondents see a brief thank-you screen confirming their input has been received and will help keep the team informed.
